Job Summary
The Registered Nurse based in the Addiction and Mental Health
Center (AMHC) outpatient clinic setting is a licensed health
professional. The nurse is a member of a multidisciplinary team
providing services under the direction of the consulting
psychiatrist. Treatment modalities used in the outpatient clinic
include: group therapy, individual therapy, family meetings,
pharmacological management, skill building groups and educational
groups. The nurse will have responsibility for leading psycho
educational groups and may facilitate other groups with other
licensed professionals from the team. Utilizing input from the
treatment team the nurse will develop, implement and update
individualized care plans. The primary patient population the nurse
works with will be adults age 65 years and older. The nurse will
also work with patients age 18 years and older who are
participating in other outpatient programs and require further
physical assessment. The nurse will collaborate with psychiatrists
in providing care for patients receiving pharmacological management
services.

Education/Training
Bachelor's Degree
Experience
3 - 5 years of Mental Health/Addiction experience preferred
License/Certification/Registration
Maryland licensure as Registered Nurse required; Certification as
Psychiatric Mental Health Nurse from American Nurses Association
preferred
CPR Certification
Knowledge, Skills & Abilities
Ability to read, write, speak and comprehend English.
Basic Computer skills to perform Data entry.
Knowledge of mental illness and chemical dependence
HIPAA policies on Confidentiality
Basic knowledge of mental health laws.
Primary Duties and Responsibilities
Clinical: Provides individual, family and group psychoeducational
services based on an understanding of normal growth and development
and mental health issues. The Registered Nurse will:
Participate in ensuring a safe and therapeutic environment.
Supervise and monitor patients' activities.
Act as a case manager for patients 65 years of age and older. Many
of these older adults have chronic medical conditions in addition
to their primary mental health diagnosis. Help older adults access
needed services, e.g. Metro Access, Manna, etc.
Track insurance authorization and do utilization review as
necessary. Makes referrals to other levels of care as
necessary.
Collaborate with inpatient staff when older adults who are in the 6
West outpatient programs are hospitalized or if there is an older
adult patient on the inpatient unit.
Provide referrals to community providers (therapist, psychiatrists,
internal medicine physicians, specialists, etc. to all 6 West
outpatients.
Collaborate with the treatment team and psychiatrist in providing
treatment to older adults.
Evaluate outpatients who are in crisis for safety in collaboration
with other team members..
Document care they provide to patients.
Collaborate with outpatient staff in providing care to all AMHC
outpatients.
Assessments: Assesses and diagnoses patient and family needs. The
assessment process focuses on the physical, psycho social realms of
care. It also identifies selfcare, educational, discharge planning
and environmental needs of the patient. These duties include:
Assessment of the patient's health care needs on a systematic and
continuous basis.
Completion of assessment of physical pain for AMHC outpatients when
needed.
Assessment of AMHC outpatients who are not feeling well physically
on request.
Administration of medications including injectable medications to
AMHC outpatients. If warranted by pt assessment, assisting pts in
preparing a weekly pill box.
Reporting any identified issues in a timely manner to the
physician.
Team: Acts as a departmental and hospital resource through informal
leadership and technical expertise. This nurse will:
Coach and/or mentor other employees.
Identify with and encourage team concepts.
Serve as integral member of the interdisciplinary team,
demonstrating an ability to effectively communicate and interact
within the team.
Participate in the orientation of new staff members, assist with
supervision of students and volunteer as needed.
Provide AMHC with support for education activities for
staff/patients.
Participate in various community mental health activities.
Consult, and develop partnerships with outside organizations and
community professionals.
Serve as team leader organizing work assignments and ensuring that
patient needs are met with supervisor is not available.
Development: Maintain current knowledge and licensure.
Identifies learning needs in self and initiates actions to address
these education needs.
Attends continuing education programs in order to maintain
certification.
Attends hospital sponsored training programs as required by the
department.
Completes all mandatory requirements, certifications, and licensure
within allotted timeframes for completion (CPR, License, PPD,
Competencies, etc.)
Organizational knowledge: Works effectively within and between
departments.
Participates on inter-departmental teams and committees,
communicating unit knowledge as appropriate.
Provides department/unit with information and knowledge acquired
during participation with interdepartmental teams and
committees.
Demonstrates a constructive approach during all interactions with
staff, supervisors, and managers both inside and outside the
unit.
Department/Unit Specific Functions and Duties
Consistently displays professionalism in interactions with patients
and families, staff and physicians.
Provides coverage as needed when there are identified needs with in
the AMHC.
