Job Description 240K PLUS PRODUCTIVITY BONUS BASED ON RVUS and
experience BC/BE Combination of Outpatient and SNF See ages 16
years and older Monday thru Friday 40-hour schedule. Schedule:
8am-12noon outpatient; 1pm " 4pm SNF 22 patients in total per day
(between both places) One Sat a month to work from 9 am to 2 pm.
(Only office patients. Usually see about 5 to 8 patient office
only) No phone call - NP will take call No nights RVU bonus above
22 patients - each additional patient is paid based on RVU based
payment model. The amount per patient increases with the level of
care patient needs. Sign on bonus Relocation assistance, PTO, 1
week of CME plus $1,500.00 per year. Malpractice, Health Insurance
(medical, dental and vision), 401k with a 3 percent match
(candidate will qualify to participate starting 2nd month of their
employment), long term disability insurance, Term life
insurance.
