Job Description:

Job Summary:#This position is responsible for the assessment, direction and supervision of participants in the Cardiopulmonary Rehab Program.# ESSENTIAL FUNCTIONS AND BASIC DUTIES Clinical:# Performs basic, routine and specialized tasks for patients# success in Cardiac and Pulmonary Rehab Programs. Evaluates medical history and current physical status of participants and assesses their baseline functional capacity and personal health factors. Obtains necessary materials from MD office for participant enrollment.# Performs new patient orientations.# Assists with navigation of resources for participants.# Develops plan of care for participants.# Independently implements plan of care utilizing principles of exercise physiology. Instructs participants in proper techniques and use of equipment.# Understands contraindications to postpone and terminate exercise.# Monitors exercise session, assessing participants for inappropriate responses to exercise. Independently adjusts participant#s treatment plan to properly progress participants and safely obtain established program and participant#s goals.# Identifies potential problems and notifies MD before critical problems appear.# Responds to emergencies which might arise prior to, during and after exercise.# Documents progress reports and discharge summaries appropriately. Education:# Provides teaching and instruction to patients, family members and staff. Independently identifies patient education needs and provides education/ resources to participant regarding disease process, prevention, and discharge planning and health maintenance. Instructs participants in proper techniques and use of equipment. Arranges for classes and participant counseling with multiple disciplines. Develops and provides in-services to hospital staff and community related to cardiac and pulmonary disease and prevention. Collaborates with multidisciplinary healthcare team to establish plan of care and continuum of care.# Develops new ideas and suggestions for program improvements. Provides input into equipment, budget needs and continuous quality improvement for the program.# Development:# Maintains current knowledge, certification, and licensure. Identifies learning needs in self and initiates actions to address these education needs. Attends continuing education programs in order to maintain certification. Attends hospital sponsored training programs as required by the department Completes all mandatory requirements, certifications, and licensure within allotted timeframe for completion (CPR, License, PPD, Competencies, Age Specifics, etc.) Organizational knowledge: Works effectively within and between departments. Participates on inter-departmental teams and committees, communicating unit knowledge as appropriate. Provides department/unit with information and knowledge acquired during participation with interdepartmental teams and committees. Demonstrates a constructive approach during all interactions with staff, supervisors, and managers both inside and outside the unit. QUALIFICATIONS Education: MS in Exercise Physiology Certification ACLS Certification ACSM Exercise specialist or Registered Clinical Exercise Physiologist (RCEP) preferred Required Knowledge: Ability to effectively communicate with all levels of internal and external customers. Basic Computer skills to perform Data entry Computerized clinical information systems: Clinicians View, Chart View. Experience conducting independent Cardiopulmonary exercise testing preferred HIPAA policies on Confidentiality Experience Required: 3 years of Clinical Exercise Physiology experience preferred

  • NRP and Fetal Monitoring Certification (L&D) for Maternal Newborn Center

  • NIH Stroke Scale Certification mandatory for ICU/IMCU, Emergency Services, Stroke Unit and Observation

  • Prefer certification in nursing specialty

  • Primary Duties and Responsibilities
  • Assessment: Collects data in an ongoing and systematic manner, focusing on the biophysical, psycho social, cognitive, self-care, educational, environmental and discharge planning needs of the patient.
  • Assesses the patient's health care needs on a systematic and continuous basis according to established time frames.

  • Identifies the biophysical, psycho-social, cultural, self-care, teaching, and continuing care needs of the patient as appropriate to patient age and diagnoses.

  • Collects data in an ongoing and systematic manner as outlined by departmental protocol.

  • Places the emphasis of assessment on the physiologic, psychological and cognitive status of the patient.

  • Reports any abnormal assessments in a timely manner to physician/mid-level provider.

  • Delegates duties and tasks to others after assessment of patient needs and capabilities, and determines the scope of responsibilities with awareness of staff members performance standards

  • Supervises members of assigned team via regular rounds and verbal interaction to validate work load, patient status and problems.

  • Planning: Formulates a goal-directed plan of care, which is prioritized and based on identified patient care needs.
  • Uses department/unit process standards as specified for planning/delivering patient care; individualizes these, holds self and co-workers accountable to them. Documents and evaluates care provided.

  • Establishes priorities of patient care on the basis of essential patient needs and available unit resources of time, personnel, equipment and supplies.

  • Maintains confidentiality of patient at all items.

  • Initiates discharge planning with initial assessment.

  • Utilizes effective communication skills to establish positive working relationship with all health team members, patients, and significant others.

  • Department/Unit Specific Functions and Duties
  • Oncology:
  • Knowledge and nursing experience in oncology

  • Satisfactory completion (within 18 months of hire) and maintains of Certification in Chemotherapy administration (ONS program)

  • ICU/IMCU:
  • Knowledge of critical care including EKG

  • Emergency Services:
  • Knowledge of emergency and critical care including EKG.

  • Technical Proficiencies include blood drawing, starting and monitoring IVs and completing EKGs.

  • Code 10: Management of the Violent Patient

  • Proficient in the use of Breathalyzer.

  • Operating Room:
  • Knowledge of critical care including EKG.

  • Assists with in room preoperative and intra-operative preparation of patients, and assists with set up of the surgical suite before, during and after a surgical procedure.

  • Ability to circulate a procedure.

  • Follows appropriate hand washing and sterilization techniques at all times.

  • Assists in preparing equipment and surgical table for procedure.

  • Moves patient from stretcher to surgical table and back with assistance.

  • Prepares and helps with patient positioning.

  • Assists with the cleaning and sterilization of Operating Room for case turnover.

  • Helps pull case carts for the following day and returns all sterile equipment to its appropriate location.

  • Recovery:
  • Knowledge of critical care including EKG.

  • Technical Proficiencies include blood drawing, starting and monitoring IVs.

  • Maternal Newborn:
  • Knowledge and nursing experience in maternal child.

  • Joint Unit:
  • Knowledge of care of a patient undergoing or receiving a joint replacement.

  • Observation Unit:
  • Knowledge of current observation criteria
